What is colorectal cancer?

Most CRCs begin as benign growths (polyps) on the inner lining of the colon/rectum. Over years, some polyps undergo genetic changes and become cancerous. Removing polyps during screening can prevent cancer.

Common polyp types include adenomas (precancerous), sessile serrated lesions (some higher-risk), and hyper plastic polyps (usually low-risk).

Who is at risk?

Risk rises with age (especially after 45). Other risks include a personal/family history of CRC or advanced polyps, inflammatory bowel disease affecting the colon, inherited syndromes (Lynch syndrome, familial adenomatous polyposis), type 2 diabetes, obesity, physical inactivity, diets high in processed/red meat and low in fiber, smoking, and heavy alcohol use. Younger-onset CRC is increasing, so symptoms in adults under 50 warrant attention.

Symptoms to

Early CRC may be silent. Seek evaluation if you have persistent rectal bleeding or blood in stool, black/tarry stools, a change in bowel habits lasting more than a few weeks, abdominal pain or cramping, unexplained weight loss, fatigue, or iron-deficiency anemia. Many of these symptoms have non-cancer causes, but they still need assessment.

Screening:

Average-risk adults should begin screening at age 45. Start earlier and screen more often if you have higher risk (family history, IBD, genetic syndromes).

Screening options include colonoscopy (every 10 years if normal), stool-based tests such as annual FIT or GFOBT and a multi-target stool DNA test every 3 years, CT colonography every 5 years, and flexible sigmoidoscopy. How CRC is diagnosed and staged

Diagnosis typically involves colonoscopy with biopsy. Pathology confirms cancer type and features. Staging evaluates spread using CT scans of chest/abdomen/pelvis; pelvic MRI or endorectal ultrasound for rectal cancer; and sometimes PET-CT. Blood work includes a complete blood count, liver function tests, and carcino embryonic antigen (CEA) for monitoring.

Staging uses the TNM system:

  • T: how deeply the tumor invades the bowel wall
  • N: lymph node involvement
  • M: distant metastases (commonly liver or lungs)

Broad stages:

  • Stage I: confined to inner layers
  • Stage II: through the wall, no nodes
  • Stage III: lymph nodes involved
  • Stage IV: distant spread

Treatment of Colorectal Cancer

Treatment is individualized based on stage, tumor location (colon vs rectum), molecular profile, and overall health. Care is best coordinated by a multidisciplinary team.

Early lesions and very early cancers: Some can be cured with endoscopic removal (polypectomy, EMR/ESD) if low risk. Surgery is advised if high-risk features are present.

Non-metastatic colon cancer (Stages I–III): Surgery (segmental colectomy with lymph node removal) is primary. Adjuvant chemotherapy is recommended for Stage III and some high-risk Stage II. Common regimens include FOLFOX, CAPOX, or single-agent fluoropyrimidine.

Non-metastatic rectal cancer: Often requires therapy before surgery (neoadjuvant chemoradiation or short-course radiation; total neoadjuvant therapy in some cases). Surgery is total mesorectal excision, with sphincter-sparing approaches when feasible. In highly selected patients with complete clinical response after neoadjuvant therapy, a structured “watch-and-wait” protocol may be considered.

Metastatic disease (Stage IV): Systemic therapy is central—chemotherapy (FOLFOX, FOLFIRI, CAPOX), targeted therapy (anti-VEGF, anti-EGFR in RAS wild-type left-sided tumors, BRAF-targeted combinations, HER2-targeted therapy), and immunotherapy for MSI-H/dMMR tumors. If metastases are limited (e.g., liver-only), surgery or ablation may lead to long-term control or cure in select patients. Palliative care and symptom management are integral at all stages.

Molecular testing and precision medicine: All CRCs should be tested for MMR/MSI and RAS/BRAF. Additional testing (HER2, NTRK fusions, others) can open targeted options. Liquid biopsy may help with monitoring and, in some settings, treatment decisions.
